The Flathead Kennel Club will be sponsoring a CAER Eye Clinic on April 27, 2024 from 8 AM to 4 PM.  Exams will be conducted by Carrie Breaux, DVM, DACVO.

The Clinic will be held at Central Valley Animal Hospital, 3650 US Hwy 2, Kalispell, MT  59901.

The cost of each exam is $55.00.

CAER exam is by appointment only.  No appointments will be made the day of the exam.  There will be NO medical exams at this clinic.  Register early to obtain desired appointment time.

Please complete the Registration Form below.  Payment is via CASH or CHECK only at the time of the exam.  US Funds only and NO Credit Cards or Debit Cards will be accepted.

All registrants must pre-register at www.ofa.org\online  Login or create a user name and password.  Create a New Application and enter your dog's information.  You must bring your case number and if possible, your printed forms, with you to your scheduled appointment.  Registration can not be completed at the time of the clinic.  PRE-REGISTRATION IS REQUIRED.

 Bring the following information with you to the exam: Dog's registered name, date of birth, sex, permanent ID number (via microchip or tattoo).
